CDEM Candidate Application

The National Fellowship of Direct-Entry Midwives appreciates your interest in becoming a Certified Direct Entry Midwife (CDEM). The first step is completing the CDEM candidate application. The initial application is a non-refundable charge of $300.00. Reduced fees may apply through the NFDEM Preceptor Bridge Program, for qualified midwives. Applications will not be processed without the required application fee. Submissions without payment will be deleted after 30 days.
